If Sam was doing drugs instead of drinking demon blood, would Dean...
a) Notice
b) Do something about it other than bitch about Sam keeping secrets from him 
And more importantly...
c)  Realize that something was majorly wrong and Sam wasn't just miraculously made whole again just because Dean came back from Hell?
